Indications
- Maintenance treatment of severe chronic obstructive pulmonary disease (COPD)
- Add on to bronchodilator treatment
- Adult patients with a history of frequent exacerbations
Pharmacology
- Phosphodiesterase-4 (PDE-4) inhibitor
- Potential antiinflammatory and antimodulatory effects
- Increased levels of intracellular cyclic AMP responsible for therapeutic actions
Dosage & Administration
- Recommended adult dose of 500 micrograms Roflumilast once daily
- May need to be taken for several weeks to achieve its effect
- Studied in clinical trials for up to one year
- Swallowed with water and taken at the same time every day
- Can be taken with or without food
Interaction
- No proven cases of hazardous interactions
- Reported interactions with ethanol, pyrimethamine, dapsone, betahistine, and salbutamol
Contraindications
- Hypersensitivity to Roflumilast or any excipients
- Moderate or severe hepatic impairment
Side Effects
- Adverse reactions including diarrhea, weight decrease, nausea, abdominal pain, and headache
- Adverse reactions mainly occurred within the first weeks of therapy
- Various rare side effects such as gynaecomastia, suicidal ideation, respiratory tract infections, and muscle spasms
Pregnancy & Lactation
- Limited data in pregnant women
- Reproductive toxicity in animal studies
- Not recommended during pregnancy or breastfeeding
Precautions & Warnings
- Informing patients about risks and safe use
- Not indicated for relief of acute bronchospasms
- Monitoring body weight
- Caution in patients with severe immunological diseases, severe acute infectious diseases, and cancers
- Increased risk of psychiatric disorders and suicidal ideation or behavior
- Reassessment in case of persistent intolerability
- No clinical data to support concomitant treatment with theophylline
- Lactose content and contraindications for patients with rare hereditary problems of galactose intolerance
Overdose Effects
- Symptoms observed at increased rate after single oral doses of 2,500 micrograms and 5,000 micrograms
- Recommended appropriate supportive medical care in case of overdose
- Haemodialysis not likely to be efficient method of removal
