Indications
- Reduction of total-C, LDL-C, ApoB, non HDL-C, and TG levels
- Increase in HDL-C
- Adjunct to diet for patients with primary hyperlipidemia
- Adjunct to diet for patients with mixed dyslipidemia
- Adjunct to diet for patients with hypertriglyceridemia

Pharmacology
- Selective and competitive inhibitor of HMG-CoA reductase
- Increases the number of hepatic LDL receptors
- Enhances uptake and catabolism of LDL
- Inhibits hepatic synthesis of VLDL
- Reduces the total number of VLDL and LDL particles
Dosage and Administration
- Usual dose range of 5-20 mg/day for pediatric patients
- Maximum recommended dose of 20 mg/day for pediatric patients
- 40 mg once daily for very high cholesterol and high risk of MI or stroke
- Recommended starting dose of 20 mg once daily for HoFH
- Consider initiation with 5 mg once daily for Asian patients
- Limit dose to 5 mg once daily with Cyclosporine
- Limit dose to 10 mg once daily with Lopinavir/Ritonavir or Atazanavir/Ritonavir
- Consider reduction in dosage when used in combination with certain lipid-lowering therapies
- For severe renal impairment, start with 5 mg once daily and not exceed 10 mg once daily
Interaction
- Increased exposure with Cyclosporine, Lopinavir/Ritonavir, Atazanavir/Ritonavir
- Prolongation of international normalized ratio (INR) with coumarin anticoagulants
- Enhanced risk of skeletal muscle effects with certain lipid-lowering therapies

- Known hypersensitivity to product components
- Liver disease
- Pregnant women and women who may become pregnant
- Nursing mothers

Pregnancy and Lactation
- Administered to women of childbearing age only when highly unlikely to conceive

- Prescribed with caution in patients with predisposing factors for myopathy
- Discontinue therapy in case of markedly elevated creatinine kinase levels
- Recommendation for liver enzyme tests
