Indications

Pharmacology
- Selective and competitive HMG-CoA reductase inhibitor
- Enhanced hepatic LDL receptors
- Inhibitor of hepatic synthesis of VLDL
Dosage & Administration
- Usual dose range 5-20 mg/day
- Maximum recommended dose 20 mg/day
- Dosage individualized according to recommended goal of therapy
- Dose adjustments at intervals of 4 weeks or more
- 40 mg once daily for very high cholesterol and high risk of MI or stroke
- Recommended starting dose for homozygous familial hypercholesterolemia 20 mg once daily
- Initiation of therapy with 5 mg once daily for Asian patients
Interaction
- Cyclosporine combination increases exposure
- Gemfibrosil combination should be avoided
- Lopinavir/Ritonavir or atazanavir/ritonavir combination increases exposure
- Coumarin anticoagulants combination prolongs INR
- Concomitant lipid-lowering therapies may increase skeletal muscle effects
Contraindications
- Known hypersensitivity to product components
- Liver disease with unexplained persistent elevations in hepatic transaminase levels
- Pregnant or potentially pregnant women
- Nursing mothers

Pregnancy & Lactation
- Prescribed with caution to women of childbearing age
- Therapy discontinued immediately if patient becomes pregnant

Precautions & Warnings
- Caution in patients with predisposing factors for myopathy
- Increased risk of myopathy with concurrent administration of other lipid-lowering therapies
- Liver enzyme tests recommended before initiation of therapy and periodically thereafter
Overdose Effects
- No specific treatment in the event of overdose
- Symptomatic treatment and supportive measures instituted
- Hemodialysis not significantly enhances clearance
