Indications
- Reduces elevated total-C levels
- Reduces LDL-C levels

- Adjunct to diet for patients with primary hyperlipidemia
- Adjunct to diet for patients with mixed dyslipidemia
- Adjunct to diet for patients with hypertriglyceredemia
- Adjunct to diet for patients with primary dysbetalipoproteinemia
- Used to slow the progression of atherosclerosis
- Pediatric use for patients 10 to 17 years of age with heterozygous familial hypercholesterolemia
- Risk reduction of Myocardial Infarction (MI), stroke, and arterial revascularization procedures
- Used for patients with homozygous familial hypercholesterolemia (HoFH)
- Adjunct to diet for patients with primary dysbetalipoproteinemia (Type III hyperlipoproteinemia)
Pharmacology
- Selective and competitive inhibitor of HMG-CoA reductase
- Increases the number of hepatic LDL receptors
- Inhibits hepatic synthesis of VLDL
Dosage & Administration
- Usual dose range is 5-20 mg/day
- Maximum recommended dose is 20 mg/day
- For very high cholesterol and high risk of MI or stroke: 40 mg once daily
- Starting dose for homozygous familial hypercholesterolemia is 20 mg once daily
- Consider 5 mg once daily for Asian patients
- Limit dose to 5 mg once daily with Cyclosporine
- Limit dose to 10 mg once daily with Lopinavir/Ritonavir or Atazanavir/Ritonavir
- Consider dose reduction when used in combination with Niacin or Fenofibrate
- Avoid combination therapy with Gemfibrozil
Interaction
- Cyclosporine increases Rovast exposure
- Gemfibrosil combination should be avoided
- Lopinavir/Ritonavir or atazanavir/ritonavir combination increases Rovast exposure
- Concomitant lipid-lowering therapies may increase the risk of skeletal muscle effects
- Combination with Coumarin anticoagulants prolongs INR
Contraindications
- Known hypersensitivity to product components
- Liver disease, which may include unexplained persistent elevations in hepatic transaminase levels
- Pregnant women and women who may become pregnant
- Nursing mothers
Side Effects
- Headache
- Myalgia
- Constipation
- Asthenia
- Abdominal pain
- Nausea
Pregnancy & Lactation
- Should be administered to women of childbearing age only when conception is highly unlikely and potential hazards have been informed
- Discontinue therapy immediately if patient becomes pregnant
- Unknown excretion in human milk
Precautions & Warnings
- Prescribed with caution in patients with predisposing factors for myopathy
- Discontinue therapy if markedly elevated creatinine kinase levels occur
- Temporarily withhold therapy in patients with acute, serious conditions suggestive of myopathy
- Recommended liver enzyme tests before initiation of therapy and periodically thereafter
Overdose Effects
- No specific treatment in the event of overdose
- Symptomatic and supportive measures should be instituted as required
- Hemodialysis does not significantly enhance clearance of Rovast
