Indications
- Adjunct to diet for reducing elevated total-C, LDL-C, ApoB, non HDL-C and TG levels
- Treatment of hypertriglyceredemia
- Treatment for primary dysbetalipoproteinemia (Type III hyperlipoproteinemia)
- Reducing LDL-C, total-C, and ApoB for patients with homozygous familial hypercholesterolemia (HoFH)
- Slowing the progression of atherosclerosis
- Risk reduction of Myocardial Infarction (MI), stroke and arterial revascularization procedures
- Reducing elevated total-C, LDL-C and ApoB in pediatric patients 10 to 17 years of age with heterozygous familial hypercholesterolemia (HeFH)
Pharmacology
- Selective and competitive inhibitor of HMG-CoA reductase
- Increases hepatic LDL receptors on the cell surface to enhance uptake and catabolism of LDL
- Inhibits hepatic synthesis of VLDL, reducing the total number of VLDL and LDL particles
Dosage & Administration
- Usual dose range of 5-20 mg/day for pediatric patients 10 to 17 years of age
- 40 mg once daily for very high cholesterol and high risk of MI or stroke
- Recommended starting dose of 20 mg once daily for homozygous familial hypercholesterolemia
- Initiation with 5 mg once daily for Asian patients
- Dose limitations with concomitant use of Cyclosporine, Lopinavir/Ritonavir, or Atazanavir/Ritonavir
- Consideration of dose reduction with concomitant use of Niacin or Fenofibrate
- Dosing considerations for patients with severe renal impairment
Interaction
- Increased exposure with concomitant use of Cyclosporine, Gemfibrosil, Lopinavir/Ritonavir or Atazanavir/Ritonavir
- Prolonged international normalized ratio (INR) with concomitant use of Coumarin anticoagulants
- Enhanced risk of skeletal muscle effects with concomitant lipid-lowering therapies
Contraindications
- Known hypersensitivity to product components
- Liver disease
- Pregnant women and women who may become pregnant
- Nursing mothers

Pregnancy & Lactation
- Administered to women of childbearing age only when conception is highly unlikely
- Discontinuation of therapy if patient becomes pregnant
- Potential excretion in human milk
Precautions & Warnings
- Prescribed with caution in patients with predisposing factors for myopathy
- Increased risk of myopathy with concurrent administration of some other lipid-lowering therapies, Gemfibrozil, Cyclosporine, Lopinavir/Ritonavir, or Atazanavir/Ritonavir
- Discontinuation of therapy in case of markedly elevated creatinine kinase levels or myopathy diagnosis
- Temporary withholding of therapy in patients with acute, serious conditions suggestive of myopathy or predisposing to the development of renal failure
- Recommendation for liver enzyme tests before and following initiation of therapy and any dose elevation
