Description
Used to reduce cholesterol saturation of bile and promote dissolution of gallstones. Reduces cholesterol secretion into bile and increases bile acid secretion rate without reduction in phospholipids.
Pharmacology
- Decreases secretion of cholesterol in bile
- Reduces cholesterol absorption
- Suppresses liver cholesterol synthesis
- Promotes formation of liquid cholesterol crystal complexes
- Improves cholestatic liver diseases by protecting cholangiocytes, stimulating hepatobilliary secretion, protecting hepatocytes against bile acid-induced apoptosis
- Completely absorbed in the upper intestine with a variable time to peak serum concentration and absorption rate
- Undergoes extraction from portal blood by liver, conjugated with amino acid, secreted into hepatic bile ducts, with small quantities appearing in circulation and very small amounts excreted into urine
- Biologic half-life ranges from 3.5-5.8 days
Dosage & Administration
- Dissolution of Gallstones: 8-12 mg/kg/day
- Primary Billiary Cirrhosis: 10-15 mg/kg/day
- Acute Viral Hepatitis: 600 mg/day
- Alcoholic Fatty Liver: 300 mg/day
- Primary Sclerosing Cholangitis: 25-30 mg/kg/day
- Dissolution of Gallstones and Non-Alcoholic Steato Hepatitis: 13-15 mg/kg/day
Interaction
Should not be used with oestrogenic hormones or bile-acid binding drugs including antacids, charcoal, and cholestyramine.
Contraindications
- Non-functioning gall-bladder
- Calcified and pigmented gallstones
- Inflammatory bowel disease

- Pruritus
Pregnancy & Lactation
Pregnancy category B. No evidence of harm reported in pregnancy. Effectively used for cholestasis treatment during the last trimester. No documented problems regarding breastfeeding.
Precautions & Warnings
Should be used cautiously in those with liver disease.
