Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Requires sexual stimulation for effectiveness
- Increases levels of cGMP in the corpus cavernosum, resulting in smooth muscle relaxation and inflow of blood into the penile tissues
- Has no effect in the absence of sexual stimulation
Indications
- Treatment of erectile dysfunction in men
- Treatment of pulmonary arterial hypertension
Mode of Action
- Involves release of nitric oxide (NO) in the corpus cavernosum during sexual stimulation
- Activates the enzyme guanylate cyclase, resulting in increased levels of cGMP and smooth muscle relaxation
- Rapidly absorbed after oral administration
Dosage & Administration
- Recommended starting dose of 50 mg
- Can be taken as needed approximately 1 hour before sexual activity
- Maximum recommended dose of 100 mg
- Maximum recommended dosing frequency is once per day
- Dosage for pulmonary arterial hypertension is 20 mg three times a day
Interactions
- Metabolism is mediated by cytochrome P450 isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce clearance
- Inducers of these isoenzymes may increase clearance
- Interaction with CYP3A4 inhibitors such as ketoconazole, itraconazole, erythromycin, saquinavir
Contraindications
- Hypersensitivity to any component of the medication
- Concomitant use with nitrates
Side Effects
- Range of body system reactions including cardiovascular, digestive, hemic and lymphatic, metabolic and nutritional, musculoskeletal, nervous, respiratory, skin and appendages, special senses, and urogenital effects
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
- No evidence of teratogenicity or embryotoxicity in animal studies
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Consideration for patients with underlying cardiovascular disease
- Sensitive patient groups such as those with left ventricular outflow obstruction or severely impaired autonomic control of blood pressure
- Considerable potential for cardiac risk in some patient groups
Overdose Effects
- Adverse events similar to lower doses but with increased incidence rates and severities
- Recommendation for standard supportive measures in cases of overdose
- Renal dialysis not expected to accelerate clearance
