Description
- Selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Inhibition of PDE5 by Medigra produces increased levels of cGMP in the corpus cavernosum
- Results in smooth muscle relaxation and inflow of blood into the penile tissues
Pharmacology
- Mechanism of Action: Involves release of nitric oxide (NO) in the corpus cavernosum during sexual stimulation
- Activation of the enzyme guanylate cyclase leading to increased levels of cGMP
- Smooth muscle relaxation in the corpus cavernosum and allowing inflow of blood
Dosage & Administration
- Recommended dose for most patients is 50 mg taken approximately 1 hour before sexual activity
- Dose may be increased to a maximum recommended dose of 100 mg or decreased to 25 mg based on effectiveness and toleration
- Maximum recommended dosing frequency is once per day
Interaction
- Metabolism is mediated by the cytochrome P450 (CYP) isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce Medigra clearance while inducers may increase clearance
Contraindications
- Hypersensitivity to any component of the medication
- Concomitant use with organic nitrates
Side Effects
- Includes cardiovascular, digestive, hemic and lymphatic, metabolic and nutritional, musculoskeletal, nervous, respiratory, skin and appendages, special senses, and urogenital effects
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
- No evidence of teratogenicity or embryotoxicity in animal studies
Precautions & Warnings
- Caution advised when PDE5 inhibitors are co-administered with alpha-blockers
- Potential for cardiac risk of sexual activity in patients with preexisting cardiovascular disease
- Particular sensitivity in patients with left ventricular outflow obstruction and severely impaired autonomic control of blood pressure
Overdose Effects
- Adverse events seen at higher incidence rates and severities
- Standard supportive measures should be adopted in cases of overdose
