Indications
- Treatment of men with erectile dysfunction
- Treatment of pulmonary arterial hypertension
Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Causes smooth muscle relaxation and inflow of blood into the penile tissues
Pharmacology
- Mechanism of Action: Involves release of nitric oxide in the corpus cavernosum during sexual stimulation
- Pharmacokinetics and Metabolism: Rapidly absorbed after oral administration, with a mean absolute bioavailability of 41%
Dosage & Administration
- Recommended dose for erectile dysfunction is 50 mg, to be taken approximately 1 hour before sexual activity
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day
Interaction
- Metabolism is mediated by cytochrome P450 isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce clearance while inducers may increase clearance
Contraindications
- Not to be used by patients with hypersensitivity to any component of the medication
- Contraindicated in patients using organic nitrates

- Pregnancy category B - no adequate and well-controlled studies in pregnant women
