Indications
- Treatment of erectile dysfunction
- Treatment of pulmonary arterial hypertension
Mechanism of Action
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Increases levels of cGMP in the corpus cavernosum
- Produces smooth muscle relaxation
- Inflow of blood into the penile tissues
Pharmacokinetics and Metabolism
- Rapidly absorbed after oral administration
- Mean absolute bioavailability of 41%
- Eliminated predominantly by hepatic metabolism
- Terminal half lives of about 4 hours
Dosage & Administration
- Recommended dose is 50 mg for most patients
- Can be taken 4 hours to 0.5 hour before sexual activity
- Maximum recommended dose of 100 mg
- Can be decreased to 25 mg based on toleration
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day
Drug Interaction
- Metabolism is mediated by cytochrome P450 isoforms
- Inhibitors of these isoenzymes may reduce clearance
- Inducers of these isoenzymes may increase clearance

- Hypersensitivity to any component
- Concomitant use with organic nitrates

Precautions & Warnings

- Caution in patients with anatomical deformation of the penis
- Not recommended to use with other treatments for erectile dysfunction
Overdose Effects
- Adverse events similar to lower doses
- Standard supportive measures should be adopted
- Renal dialysis is not expected to accelerate clearance
