Indications
- Treatment of paroxysmal atrial fibrillation/flutter (PAF)
- Treatment of paroxysmal supraventricular tachycardia (PSVT)
- Treatment of ventricular arrhythmias
Pharmacology
- Slows influx of sodium ions into cardiac muscle cells
- Decreases excitability of cells
- Selective for cells with a high rate
- Blocks normal cells more than class Ia or Ib
- Additional activity as a beta-adrenergic blocker
Dosage & Administration
- Individually titrated dose
- Initiate therapy with 150 mg every eight hours
- Dosage increase at minimum of 3 to 4 day intervals
- Maximum recommended dosage of 900 mg per day
Interaction
- Metabolized by CYP2D6, CYP1A2, and CYP3A4
- Inhibited by drugs such as desipramine, paroxetine, ritonavir
- Inhibits CYP2D6
- Potential drug interactions with desipramine, imipramine, haloperidol, venlafaxine
Contraindications
- Uncontrolled congestive heart failure
- Cardiogenic shock
- Sinoatrial, atrioventricular and intraventricular disorders of impulse generation and/or conduction
- Bradycardia
- Marked hypotension
- Bronchospastic disorders
- Electrolyte imbalance
- Known hypersensitivity to the drug
