Indications
- Monotherapy and Combination Therapy
- Adjunct to diet and exercise

- Not for type 1 diabetes
- Not for diabetic ketoacidosis
Pharmacology
- DPP-4 inhibitor
- Slows inactivation of incretin hormones
- Increases and prolongs action of hormones
- Regulates glucose homeostasis
- Increases insulin release
- Decreases glucagon levels
Dosage & Administration
- Recommended dose: 50 mg twice a day
- Recommended dose: 100 mg once daily
- Can be taken with or without food
Interaction
- No meaningful alteration in pharmacokinetics of certain drugs
- Slightly increases concentration of Digoxin
- No dose adjustment required
Contraindications
- History of serious hypersensitivity reaction to sitagliptin
- History of anaphylaxis or angioedema
Side Effects
- Common adverse reactions: headache, upper respiratory tract infection, nasopharyngitis
- Hypoglycemia may occur in some patients
Pregnancy & Lactation
- Pregnancy Category B
- Reproduction studies in rats and rabbits
- Use during pregnancy only if clearly needed
- Caution in nursing women
Precautions & Warnings
- Prompt discontinuation if pancreatitis is suspected
- Dosage adjustment in patients with renal insufficiency
- Use with medications known to cause Hypoglycemia
- Hypersensitivity Reactions
- Reports of serious hypersensitivity reactions in patients
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- Elderly patients with decreased renal function
- Dose adjustment in elderly patients
- Dose adjustment in patients with renal insufficiency
- Dose adjustment in patients with ESRD
Overdose Effects
- Maximal mean increases in QTc observed
- No experience with doses above 800 mg in clinical studies
- Usual supportive measures in the event of an overdose
- Modestly dialyzable
- Prolonged hemodialysis may be considered if clinically appropriate
