Indications
- Adjunct to diet and exercise
- Improves glycemic control
- Type 2 diabetes mellitus
- Not for type 1 diabetes
- Not for diabetic ketoacidosis
- Not studied in patients with pancreatitis
Pharmacology
- DPP-4 inhibitor
- Slows inactivation of incretin hormones
- Increases and prolongs action of incretin hormones
- Regulates glucose homeostasis
- Increases insulin release
- Decreases glucagon levels
Dosage & Administration
- Recommended dose: 50mg twice a day or 100mg once daily
- Can be taken with or without food
Interaction
- Did not meaningfully alter the pharmacokinetics of various drugs
- Slightly increases mean Digoxin concentration
Contraindications
- History of serious hypersensitivity reaction to sitagliptin
Side Effects
- Common adverse reactions include headache, upper respiratory tract infection, nasopharyngitis
- Hypoglycemia may occur in patients treated with the combination to Sucosit and sulfonylurea, and add on to insulin
Pregnancy & Lactation
- Pregnancy Category B
- Use during pregnancy only if clearly needed
- Secreted in the milk of lactating rats
- Caution when administered to a nursing woman
Precautions & Warnings
- Promptly discontinue if pancreatitis is suspected
- Dosage adjustment in patients with renal insufficiency
- Usable with medications known to cause hypoglycemia
- Hypersensitivity reactions may occur
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- Careful dose selection in the elderly
- Different dosage for patients with renal insufficiency
Overdose Effects
- Maximal mean increases in QTc
- No dose-related clinical adverse reactions observed
- Modestly dialyzable
- Usual supportive measures in case of overdose
- Not known if dialyzable by peritoneal dialysis
