Sotalex 80 mg (Tablet)

Indications

• Treatment of life-threatening arrhythmias
• Prophylaxis of paroxysmal atrial tachycardia or fibrillation
• Maintenance of sinus rhythm following cardioversion
• Symptomatic non-sustained ventricular tachyarrhythmias

Pharmacology

• Non-cardioselective beta-blocker
• Increases sinus cycle length
• Decreases AV nodal conduction
• Prolongs AV monophasic action potentials

Dosage & Administration

• Initial dosage of 80 mg daily
• Usual dose of 160-320 mg daily
• Recommended dose adjustments for renal impairment
• Higher doses for life-threatening ventricular arrhythmias

Interaction

• Interactions with clonidine
• Use caution with drugs prolonging OT interval
• Monitoring required with certain drug combinations

Contraindications

• Not to be given to patients with sinus or bradycardia
• Avoid in long QT syndromes and torsades de pointes
• Contraindicated in uncontrolled heart failure, cardiogenic shock

Side Effects

• Class I and II effects
• Cardiac action potential duration prolongation

Pregnancy & Lactation

• Avoid use throughout pregnancy if possible
• Potential fetal bradycardia
• Avoid breastfeeding during treatment

Precautions & Warnings

• Not indicated for serious kidney problems or lung diseases
• Not for very slow heart rate without artificial pacemaker
• Special consideration for patients with various conditions

Overdose Effects

• Excessive bradycardia and hypotension
• Causes congestive heart failure and bronchospasm

Therapeutic Class

• Anti adrenergic agent (Beta blockers)
