Indications
- Monotherapy and Combination Therapy
- Improves glycemic control in adults with type 2 diabetes mellitus
- Not for use in type 1 diabetes
Pharmacology
- DPP-4 inhibitor
- Slows inactivation of incretin hormones
- Increases and prolongs action of incretin hormones
- Regulates glucose homeostasis
- Increases insulin release and decreases glucagon levels
Dosage & Administration
- Recommended dose of 50 mg twice a day or 100 mg once daily
- Can be taken with or without food
Interaction
- Does not meaningfully alter the pharmacokinetics of certain drugs
- Slightly increases mean concentration of Digoxin
- No dose adjustment required for Digoxin
Contraindications
- History of serious hypersensitivity reaction to sitagliptin
Side Effects
- Common adverse reactions include headache, upper respiratory tract infection, and nasopharyngitis
- Hypoglycemia may occur in patients treated with combination to Glucodip and sulfonylurea and add on to insulin
Pregnancy & Lactation
- Pregnancy Category B
- Should be used during pregnancy only if clearly needed
- Caution should be exercised when administered to a nursing woman
Precautions & Warnings
- Prompt discontinuation if pancreatitis is suspected
- Dosage adjustment recommended in patients with renal insufficiency
- Careful dose selection in elderly patients with decreased renal function
- Caution with medications known to cause hypoglycemia
- Reports of serious hypersensitivity reactions in patients treated with Glucodip
Use in Special Populations
- Safety and effectiveness not established in pediatric patients under 18 years of age
- Care with dose selection in elderly patients with decreased renal function
Overdose Effects
- Maximal mean increases in QTc observed at high doses
- Usual supportive measures to be employed in case of overdose
- Modestly dialyzable
