Indications
- Adjunct to diet and exercise for glycemic control in adults with type 2 diabetes mellitus
Pharmacology
- Sitagliptin is a DPP-4 inhibitor
- Increases insulin release
- Decreases glucagon levels in a glucose-dependent manner
- Part of an endogenous system for glucose regulation
Dosage & Administration
- Recommended dose of sitagliptin: 50 mg twice a day or 100 mg once daily
- Can be taken with or without food
Interaction
- Does not meaningfully alter the pharmacokinetics of certain drugs
- Slightly increases the mean concentration of Digoxin
- No dose adjustment required for Digoxin
Contraindications
- History of serious hypersensitivity reaction to sitagliptin (anaphylaxis or angioedema)
Side Effects
- Most common adverse reactions: headache, upper respiratory tract infection, nasopharyngitis
- Hypoglycemia may occur in patients treated with Glucodip and sulfonylurea/add-on to insulin
Pregnancy & Lactation
- Pregnancy Category B
- Caution advised when administered to a nursing woman
Precautions & Warnings
- Prompt discontinuation if pancreatitis is suspected
- Dosage adjustment recommended in patients with renal insufficiency
- Increased risk of hypoglycemia with certain medications
- Reports of serious hypersensitivity reactions

- Safety and effectiveness not established in pediatric patients
- Careful dose selection in elderly patients

Overdose Effects
- Maximal mean increases in QTc observed at high doses
- Supportive measures recommended in the event of an overdose
