Indications
- Adjunct to diet and exercise
- Improved glycemic control in adults with type 2 diabetes mellitus
Pharmacology
- Sitagliptin is a DPP-4 inhibitor
- Slows inactivation of incretin hormones
- Increases and prolongs action of incretin hormones
- Regulates glucose homeostasis
- Increases insulin release
- Decreases glucagon levels
- Selective for DPP-4
Dosage & Administration
- Recommended dose of sitagliptin is 50 mg twice a day
- Can be taken with or without food
Interaction
- No meaningful alteration in pharmacokinetics of other drugs
- Slight increase in mean Digoxin concentration
Contraindications
- History of serious hypersensitivity reaction to sitagliptin
Side Effects
- Common adverse reactions include headache, upper respiratory tract infection, and nasopharyngitis
- Hypoglycemia may occur in patients treated with the combination to Glucodip and sulfonylurea and add on to insulin
Pregnancy & Lactation
- Pregnancy Category B
- Use during pregnancy only if clearly needed
- Secreted in the milk of lactating rats at a specific ratio
- Caution in nursing women
Precautions & Warnings
- Prompt discontinuation if pancreatitis is suspected
- Dosage adjustment in patients with renal insufficiency
- Caution with medications known to cause hypoglycemia
- Reports of serious hypersensitivity reactions
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- Careful dose selection in elderly patients with decreased renal function
Overdose Effects
- Maximal mean increases in QTc observed at high doses
- Employ usual supportive measures in case of overdose
