Indications

  • Adjunct to diet and exercise for glycemic control in type 2 diabetes mellitus
  • Not indicated for type 1 diabetes or diabetic ketoacidosis treatment
  • Not studied in patients with a history of pancreatitis

Pharmacology

  • Combines sitagliptin and metformin as antihyperglycemic agents
  • Sitagliptin: DPP-4 inhibitor affecting incretin hormones
  • Metformin: Decreases hepatic glucose production and increases peripheral glucose uptake

Dosage & Administration

  • Individualized dosage based on patient's regimen and tolerability
  • Initial combination therapy based on healthcare provider's discretion
  • Starting dose and gradual escalation with meals to reduce GI side effects

Interaction

  • Cationic drugs caution for renal tubular secretion
  • Metformin may decrease phenprocoumon's anticoagulant effect
  • Levothyroxine can reduce metformin's hypoglycemic effect

Contraindications

  • Contradicted in patients with renal disease or dysfunction
  • Acute or chronic metabolic acidosis including diabetic ketoacidosis
  • History of serious hypersensitivity reaction to the tablet or sitagliptin

Side Effects

  • Common adverse reactions include diarrhea, upper respiratory tract infection, and headache
  • Hypoglycemia reported in patients treated with insulin and metformin
  • Nasopharyngitis reported in patients treated with sitagliptin monotherapy

Pregnancy & Lactation

  • Pregnancy Category B - safety not well-studied in pregnant women
  • Caution advised when administered to a nursing woman

Precautions & Warnings

  • Lactic acidosis risk with conditions like sepsis, dehydration, and renal impairment
  • Hepatic disease caution, monitor renal and pancreatic function
  • Potential decrease in vitamin B12 serum levels with long-term use

Overdose Effects

  • Supportive measures for overdose, consider hemodialysis for metformin
  • Pancreatitis may occur in the context of a metformin overdose
