Indications
- Adjunct to diet and exercise for type 2 diabetes mellitus
- Improves glycemic control in adults
- Appropriate for combination therapy with sitagliptin and metformin
- Not for type 1 diabetes or diabetic ketoacidosis
Pharmacology
- Combines sitagliptin and metformin for antihyperglycemic action
- Sitagliptin is a DPP-4 inhibitor
- Metformin HCl is a biguanide class member
- Mechanism of action involves incretin hormone regulation
Dosage & Administration
- Individualized dosage based on patient's regimen
- Maximum recommended daily dose of 100 mg sitagliptin and 2000 mg metformin
- Initial combination therapy or maintenance should be individualized
- Gradual dose escalation with meals to reduce GI side effects
Interaction
- Caution with cationic drugs eliminated by renal tubular secretion
- Metformin may decrease anticoagulant effect of phenprocoumon
- Levothyroxine can reduce hypoglycemic effect of metformin
Contraindications
- Renal disease or dysfunction
- Acute or chronic metabolic acidosis
- History of serious hypersensitivity reaction to the tablet or sitagliptin
- Discontinue temporarily for patients undergoing radiologic studies with iodinated contrast materials
Side Effects
- Common adverse reactions include diarrhea, upper respiratory tract infection, and headache
- Hypoglycemia reported in combination with sulfonylurea and insulin
- Nasopharyngitis reported in sitagliptin monotherapy
- Common adverse reactions due to metformin initiation include diarrhea, nausea/vomiting, and headache
Pregnancy & Lactation
- Pregnancy Category B
- Safety in pregnant women not well-studied
- Caution advised during nursing
Precautions & Warnings
- Risk of lactic acidosis with metformin accumulation
- Monitor TSH levels in patients with hypothyroidism
- Long-term treatment associated with decreased vitamin B12 levels
Overdose Effects
- Usual supportive measures for sitagliptin overdose
- Metformin overdose may require hemodialysis for removal
