Description

  • Used in the management of heart failure
  • Indicated in atrial fibrillation with an uncontrolled ventricular rate
  • Treatment for acute/left ventricular failure
  • Suitable for chronic/left ventricular failure and congestive heart failure especially caused by hypertensive valvular disease or ischemic heart disease

Pharmacology

  • Positive inotropic activity
  • Reduction in conductivity of the heart
  • Direct action on vascular smooth muscle
  • Indirect effects mediated primarily by the autonomic nervous system
  • Increase in vagal activity

Dosage & Administration

  • Rapid digitalization: 1-1.5 mg in divided doses over 24 hours
  • Less urgent digitalization: 250-500 micrograms daily
  • Maintenance: 62.5-500 micrograms daily
  • Usual range: 125-250 micrograms daily
  • Appropriate dose for the elderly
