Therapeutic Class

  • Combination Oral hypoglycemic preparations

Mode of Action

  • Combines two antihyperglycemic agents with complementary mechanisms of action
  • Sitagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor
  • Metformin HCl is a member of the biguanide class
  • Sitagliptin slows the inactivation of incretin hormones (GLP-1 and GIP)

Indications

  • Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus
  • Appropriate for use when treatment with both sitagliptin and metformin is suitable
  • Not indicated for type 1 diabetes or for the treatment of diabetic ketoacidosis

Dosage & Administration

  • Dosage should be individualized based on the patient's current regimen, effectiveness, and tolerability
  • Initial combination therapy should be individualized and left to the discretion of the health care provider
  • The tablet should generally be given twice daily with meals
  • Recommended starting doses for patients not currently treated with metformin and those already treated with metformin
  • Extended-release tablet should be administered once daily with a meal, preferably in the evening

Interaction

  • Use caution with cationic drugs eliminated by renal tubular secretion
  • Metformin may decrease the anticoagulant effect of phenprocoumon

Contraindications

  • Contradicted in patients with renal disease or renal dysfunction
  • Acute or chronic metabolic acidosis
  • History of a serious hypersensitivity reaction to the tablet or sitagliptin
  • Temporarily discontinued in patients undergoing radiologic studies involving intravascular administration of iodinated contrast materials

Side Effects

  • Common adverse reactions include diarrhea, upper respiratory tract infection, and headache
  • Hypoglycemia reported in combination with sulfonylurea and metformin, and insulin and metformin

Pregnancy & Lactation

  • Pregnancy Category B
  • Safety in pregnant women is not known
  • Caution should be exercised when administered to a nursing woman

Precautions & Warnings

  • Lactic acidosis risk increases with certain conditions
  • Regular monitoring of thyroid-stimulating hormone (TSH) levels is recommended in patients with hypothyroidism
  • Long-term treatment has been associated with a decrease in vitamin B12 serum levels

Overdose Effects

  • Supportive measures should be employed in case of an overdose
  • Sitagliptin is modestly dialyzable and prolonged hemodialysis may be considered if clinically appropriate
  • Hemodialysis may be useful for removal of accumulated drug from patients in whom metformin overdosage is suspected

Storage Conditions

  • Store below 25°C in a dry place away from light
  • Keep the medicine out of the reach of children
  • Do not use after the expiry date
