Indications

  • Adjunct to diet and exercise for type 2 diabetes mellitus
  • Improves glycemic control

Pharmacology

  • Combines sitagliptin and metformin
  • Dipeptidyl peptidase-4 (DPP-4) inhibitor
  • Biguanide class of drugs
  • Slows inactivation of incretin hormones
  • Regulates glucose homeostasis
  • Increases insulin release and decreases glucagon levels

Dosage & Administration

  • Individualized dosage based on patient's current regimen
  • Maximum recommended daily dose of 100mg sitagliptin and 2000mg metformin
  • Twice daily with meals
  • Gradual dose escalation to reduce gastrointestinal side effects
  • Adjust dosing based on effectiveness and tolerability
  • Administer extended-release tablet once daily with a meal
  • Consideration for co-administration with insulin secretagogue or insulin

Interaction

  • Use with caution with cationic drugs eliminated by renal tubular secretion
  • Metformin may decrease the anticoagulant effect of phenprocoumon
  • Levothyroxine can reduce the hypoglycemic effect of metformin

Contraindications

  • Renal disease or renal dysfunction
  • Acute or chronic metabolic acidosis
  • History of serious hypersensitivity reaction
  • Use temporarily discontinued for patients undergoing radiologic studies with iodinated contrast materials

Side Effects

  • Common adverse reactions include diarrhea, upper respiratory tract infection, and headache
  • Hypoglycemia potential with combination therapy
  • Nasopharyngitis reported with sitagliptin monotherapy
  • Common adverse reactions due to initiation of metformin therapy include diarrhea, nausea/vomiting, flatulence, and headache

Pregnancy & Lactation

  • Pregnancy Category B
  • Caution recommended during lactation due to unknown excretion of sitagliptin in human milk

Precautions & Warnings

  • Lactic acidosis risk with metformin accumulation
  • Monitoring of thyroid-stimulating hormone (TSH) levels recommended in patients with hypothyroidism
  • Long-term treatment associated with decrease in vitamin B12 serum levels
  • Postmarketing reports of acute renal failure, pancreatitis, and allergic reactions

Overdose Effects

  • Usual supportive measures for sitagliptin overdose
  • Metformin is dialyzable with potential for hemodialysis in overdosage
