Indications
- Improves glycemic control in adults with type 2 diabetes mellitus
- Adjunct to diet and exercise
- Not for type 1 diabetes or diabetic ketoacidosis
- Not studied in patients with a history of pancreatitis
Pharmacology
- Combines sitagliptin and metformin for glycemic control
- Sitagliptin: DPP-4 inhibitor, slows incretin hormone inactivation
- Metformin: decreases hepatic glucose production, increases peripheral glucose uptake
Dosage & Administration
- Individualized dosage based on current regimen, effectiveness, and tolerability
- Initial or maintenance therapy should be individualized
- Starting dose varies based on current treatment
- Film-coated tablet: generally given twice daily with meals
- Extended-release tablet: administer once daily with a meal, gradually escalate the dose
Interaction
- Cationic drugs: use with caution if eliminated by renal tubular secretion
- Phenprocoumon: metformin may decrease anticoagulant effect
- Levothyroxine: may reduce hypoglycemic effect of metformin
Contraindications
- Renal disease or dysfunction
- Metabolic acidosis, diabetic ketoacidosis
- History of serious hypersensitivity reaction
Side Effects
- Common adverse reactions: diarrhea, upper respiratory tract infection, headache
Pregnancy & Lactation
- Pregnancy Category B
- Use during pregnancy only if clearly needed
- Exercise caution when administered to a nursing woman
Precautions & Warnings
- Risk of lactic acidosis, regular monitoring recommended
- Decrease in vitamin B12 levels with long-term metformin use
- Postmarketing reports of acute renal failure, pancreatitis, and allergic reactions
- Evaluate patients for evidence of ketoacidosis or lactic acidosis
- Consider lower dose of insulin or insulin secretagogue when used in combination
Overdose Effects
- Usual supportive measures for sitagliptin overdose
- Metformin overdose: dialyzable; consider hemodialysis for removal
