Indications
- Hypertension
Pharmacology
- Centrally acting sympatholytic agent
- Antihypertensive agent
- Prodrug
- Alpha-2 adrenergic receptor agonist
- Reduction of vasoconstrictor adrenergic signals
Dosage & Administration
- Starting dosage of 250 mg two or three times a day
- Maintenance dosage of 500 mg to 2 g in two to four doses
- Pediatric dosage based on 10 mg/kg of body weight daily
Interaction
- Potentiation of antihypertensive effect with other antihypertensive drugs
- Reduced doses of anesthetics may be required
- Careful monitoring for symptoms of lithium toxicity when used with lithium
- Not recommended to be given with ferrous sulfate or ferrous gluconate
Contraindications
- Active hepatic disease
- Hypersensitivity to any component
- Therapy with monoamine oxidase (MAO) inhibitors

Precautions & Warnings
- Caution in patients with history of liver disease or dysfunction
- Clinical edema or weight gain may be controlled by diuretic
- Not to be continued if edema progresses or signs of heart failure appear
- Recurrence of hypertension after dialysis
- Involuntary movements in severe bilateral cerebrovascular disease
Use in Special Populations
- No well-controlled clinical trials in pediatric patients
- Pediatric dosing supported by evidence from published literature
Overdose Effects
- Acute hypotension
- Excessive sedation
- Bradycardia
- Dizziness
- Symptomatic and supportive measures should be employed in case of overdose
