Indications
- Treatment of chronic hepatitis C (CHC) infection
- Efficacy confirmed in subjects with HCV genotype 1, 2, 3 or 4 infection
- Suitable for patients with hepatocellular carcinoma meeting Milan criteria and HCV/HIV-1 co-infection
- Not recommended for monotherapy of CHC
Pharmacology
- Nucleotide analog inhibitor of HCV NS5B
- Acts as a chain terminator
- Prevents HCV viral replication by binding to the active site motif in HCV NS5B polymerase
Dosage & Administration
- One 400 mg tablet taken once daily with or without food
- Used in combination with Ribavirin or Pegylated Interferon and Ribavirin for the treatment of CHC
- Recommended combination therapy for different HCV genotypes
Interaction
- Reduced therapeutic effect with potent P-gp inducers
- May cause serious symptomatic bradycardia when co-administered with amiodarone
- Interaction with P-gp &/or BCRP inhibitors
Contraindications
- Contraindicated in pregnant women and men with pregnant partners due to risks for birth defects and fetal death associated with Ribavirin
Side Effects
- Common adverse events include fatigue, headache, nausea, insomnia, anemia, pruritus, asthenia, rash, and more
Pregnancy & Lactation
- Pregnancy Category B: No adequate and well-controlled studies in pregnant women
- Unknown presence of Sofosbuvir and its metabolites in human breast milk
Precautions & Warnings
- Risk of serious symptomatic bradycardia with amiodarone co-administration
- Cardiac monitoring recommended in certain patients
Use in Special Populations
- Safety and effectiveness not established in children less than 18 years of age
- Response rates similar in subjects aged 65 and over
- No dose adjustment warranted in geriatric patients
Overdose Effects
- No specific antidote available for overdose treatment
- General supportive measures and monitoring of vital signs recommended
