Indications

Description

Long-acting aldosterone antagonist. Specific pharmacologic antagonist of aldosterone, causing increased sodium and water excretion while retaining potassium and magnesium.

Pharmacology

Aldosterone antagonist acting through competitive binding of receptors at the aldosterone-dependent exchange site, causing increased sodium and water excretion while retaining potassium. Acts as a diuretic and antihypertensive drug.

Dosage & Administration

  • Initial daily dosage of 100mg for adults with edema
  • Doses of 100-400mg for primary hyperaldosteronism
  • Initial dosage of 50-100mg for essential hypertension
  • 25-100mg dosage for treating diuretic-induced hypokalemia

Interactions

  • Severe hyperkalemia with ACE inhibitors
  • Potentiation of orthostatic hypotension with alcohol, barbiturates, or narcotics
  • Intensified electrolyte depletion with corticosteroids, ACTH
  • Increased digoxin half-life

Contraindications

  • Acute renal insufficiency
  • Significant impairment of renal function
  • Anuria
  • Hyperkalemia
  • Sensitivity to Spironolactone

Precautions & Warnings

Patients receiving diuretic therapy should be observed for evidence of fluid or electrolyte imbalance. Hyperkalemia may occur and can cause cardiac irregularities.
