Indications

Pharmacology
Tadalafil is a selective phosphodiesterase type 5 (PDE5) inhibitor which increases cGMP in smooth muscle cells, leading to smooth muscle relaxation and increased blood flow into the corpus cavernosum, causing penile erection. It also relaxes smooth muscles in the prostate and bladder.
Dosage & Administration
- Erectile Dysfunction: Recommended starting dose is 10 mg, with possible adjustments to 20 mg or 5 mg. Maximum dosing frequency is once daily. Effective for up to 36 hours.
- Benign Prostatic Hyperplasia: Recommended dose is 5 mg taken daily.
- Combined Erectile Dysfunction and BPH: Recommended dose is 5 mg taken daily.
Interaction
May interact with Nitrates, Alpha adrenergic blockers, Antihypertensives, Alcohol, Antacids, Ketoconazole, Ritonavir, Erythromycin, Itraconazole, Grapefruit juice, other HIV protease inhibitors, Rifampin, Carbamazepine, Phenytoin & Phenobarbital.

Side Effects
- Common: Headache, Dyspepsia, Back pain, Myalgia, Nasal pharyngitis, Nasal congestion
- Rare: Change in Color Vision, Sudden vision loss, Hearing loss, Stevens-Johnson Syndrome, Exfoliative dermatitis, Angina, Stroke, Myocardial infarction, Severe hypotension, Tachycardia
Pregnancy & Lactation
Tadalafil has been assigned to pregnancy category B by the USFDA. Caution should be used when administering tadalafil to nursing women.
