Indications
- Erectile Dysfunction (ED)
- Benign Prostatic Hyperplasia (BPH)
- Both Erectile Dysfunction and signs and symptoms of Benign Prostatic Hyperplasia
Pharmacology
- Tadalafil is a selective phosphodiesterase type 5 (PDE5) inhibitor
- Increases cGMP in smooth muscle cells
- Causes smooth muscle relaxation and increased blood flow into the corpus cavernosum
- PDE5 also present in smooth muscles of the prostate and bladder wall
- Inhibiting PDE5 increases cGMP concentrations leading to relaxation of smooth muscle in the prostate and bladder
- Smooth muscle relaxation may improve blood flow to the urinary tract
- Widens the opening of the bladder neck, resulting in improved voiding
Dosage & Administration
- Recommended starting dose for Erectile Dysfunction: 10 mg
- Recommended dose for Benign prostatic hyperplasia: 5 mg taken daily
- Recommended dose for Combined Erectile Dysfunction and Benign prostatic hyperplasia: 5 mg daily
- Maximum dosing frequency is once daily
- Tadalafil is effective for up to 36 hours
Interaction
- May interact with Nitrates, Alpha adrenergic blockers, Antihypertensives, Alcohol, Antacids, Ketoconazole, Ritonavir, Erythromycin, Itraconazole, Grapefruit juice, Rifampin, Carbamazepine, Phenytoin, Phenobarbital
Contraindications
- Use of Nitrates may increase hypotensive effects
- Hypersensitivity reactions to Tadalafil
Side Effects
- Common side effects: Headache, Dyspepsia, Back pain, Myalgia, Nasal pharyngitis, Nasal congestion
- Rare side effects: Change in Color Vision, Sudden vision loss, Hearing loss, Stevens-Johnson Syndrome, Exfoliative dermatitis, Angina, Stroke, Myocardial infarction, Severe hypotension, Tachycardia
Pregnancy & Lactation
- Tadalafil assigned to pregnancy category B by the USFDA
- Recommended for use during pregnancy when benefit outweighs risk
- No data on the excretion of Tadalafil in human milk
- Caution should be used when administering tadalafil to nursing women
Precautions & Warnings
- Precaution should be taken in conditions such as Angina, renal impairment, hepatic impairment, bleeding concomitant with Nitrates, Alpha Blockers, Alcohol, CYP3A4 Inhibitors, other PDE5 inhibitors
