Indications
- Hypertension

- Selective ß1 blocker

- Almost complete absorption from gastrointestinal tract

- Balanced clearance between renal elimination and hepatic metabolism
- Elimination half-life of 10-12 hours
Dosage & Administration
- Usual starting dose of 5 mg once daily for adults
- Increased to 10 mg and then to 20 mg once daily if necessary
- Appropriate titration interval of 2 weeks
- Initial daily dose of 5 mg for patients with renal or hepatic impairment
- No need to adjust dose in the elderly unless significant renal or hepatic dysfunction
- Safety and effectiveness not established in children
Interaction
- Not to be combined with other β-blocking agents
- Careful monitoring with catecholamine-depleting drugs
- Exacerbation of rebound hypertension with centrally active antihypertensive agents
- Caution with myocardial depressants or inhibitors of A-V conduction
- Potential severe hypotension, bradycardia, and cardiac failure with combined use of β-blockers and calcium channel blockers
Contraindications
- Cardiogenic shock
- Overt heart failure
- Second or third degree A-V block
- Right ventricular failure secondary to pulmonary hypertension
- Sinus bradycardia
Side Effects
- Fatigue
- Dizziness
- Headache
- Disturbances of the gut such as nausea, vomiting, diarrhea, constipation, or abdominal pain
- Cold or numb extremities
- Muscle weakness or cramps
- Breathing difficulties due to a narrowing of the airways (bronchospasm) in people with asthma or COPD
Pregnancy & Lactation
- No teratogenic effects observed in rats at recommended human daily doses
- Potential benefit justifies potential risk to the fetus in pregnancy
- Small amounts detected in the milk of lactating rats
Precautions & Warnings
- Use caution in adjusting the dose in patients with renal or hepatic impairment
- Patients with a history of severe anaphylactic reaction may be more reactive to repeated challenge
- Unresponsive to usual doses of epinephrine
