Indications
- Treatment of functional symptoms of Benign Prostatic Hyperplasia (BPH)
Pharmacology
- Selective alpha1 adrenoceptor blocking agent
- Relaxes smooth muscle in the bladder neck and prostate
- Improves urine flow rate and reduces BPH symptoms
- High plasma protein binding (94% to 99%)
- Extensively metabolized by cytochrome P 450 enzymes
- Long elimination half-life (5 to 15 hours)
Dosage & Administration
- Tamsulosin Hydrochloride 0.4 mg daily, to be taken after meal at night
- Dose may be increased to 0.8 mg after 2 to 4 weeks if necessary
- Swallow whole with a glass of water
- Should not be crunched or chewed
Interaction
- Concurrent administration with other alfa1-adrenoceptor antagonists can lead to hypotensive effects
- No interactions seen with atenolol, enalapril, or nifedipine
- Cimetidine increases plasma levels, frusemide decreases plasma levels
- No interactions at the level of hepatic metabolism with amitriptyline, salbutamol, glibenclamide, and finasteride
- Diclofenac and warfarin may increase elimination rate
Contraindications
- Hypersensitivity to tamsulosin hydrochloride
- History of orthostatic hypotension
- Severe hepatic insufficiency
- Possible reduction in blood pressure leading to syncope
- Caution required in situations where injury could result
- Examine patient to exclude other conditions causing BPH symptoms
- Digital rectal examination and PSA determination prior to treatment and at regular intervals

Pregnancy & Lactation
- Not recommended for use during pregnancy and lactation
Precautions & Warnings
- Transient postural symptoms may occur during orthostatic provocation testing
- Not advised in patients with micturition syncope
- Unclear effect on ability to drive or operate machines
- Patients should be aware of the possibility of dizziness
Overdose Effects
- Acute hypotension likely in case of overdosage
- Cardiovascular support required
- Renal function monitoring and general supportive measures
- Gastric lavage and activated charcoal administration in large overdoses
