Indications

  • Treatment of essential hypertension in adults
  • Reduction of cardiovascular morbidity in adults with atherothrombotic cardiovascular disease
  • Reduction of cardiovascular morbidity in adults with type 2 diabetes mellitus and documented target organ damage

Pharmacology

  • Telmisartan is a non-peptide angiotensin-ll receptor antagonist
  • Blocks the vasoconstrictor and aldosterone secreting effects of angiotensin-ll
  • Has much greater affinity for the AT1 receptor than for the AT2 receptor

Dosage & Administration

  • Usual starting dose of 40 mg once a day
  • Blood pressure response is dose-related over the range of 20 to 80 mg
  • Most antihypertensive effect is apparent within 2 weeks
  • Maximal reduction is generally attained after 4 weeks
  • No initial dosage adjustment necessary for elderly patients or patients with renal impairment
  • Initial therapy not recommended in patients ≥75 years old or with hepatic impairment

Interaction

  • Increased risk of renal impairment and loss of antihypertensive effect with NSAIDs
  • Co-administration with aliskiren in patients with diabetes should be avoided

Contraindications

  • Known hypersensitivity to this product or any of its components

Side Effects

  • Common side effects in hypertensive patients include sinus pain, congestion (sinusitis), and back pain
  • Common side effects in patients of cardiovascular risk reduction include intermittent claudication and skin ulcer

Pregnancy & Lactation

  • Pregnancy Category C (first trimester) and D (second and third trimester)
  • Potential adverse effects on the nursing infant

Precautions & Warnings

  • Avoid fetal or neonatal exposure
  • Hypotension
  • Monitor carefully in patients with impaired hepatic or renal function
  • Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker

Overdose Effects

  • Likely manifestations of hypotension, dizziness, and tachycardia
  • Possible bradycardia, increase in serum creatinine, and acute renal failure
