Indications
- Treatment of hypertension
- Lowering blood pressure
- Reducing risk of cardiovascular events
- Use alone or with other antihypertensive agents
Pharmacology
- Telmisartan blocks AT1 receptor in many tissues
- Hydrochlorothiazide is a thiazide diuretic
- Increases plasma renin activity and aldosterone secretion
- Reduces plasma volume
- Mechanism of antihypertensive effect not fully understood
Dosage & Administration
- Telmisartan monotherapy 80 mg once daily
- Telmisartan and Hydrochlorothiazide 80 mg/12.5 mg once daily
- Dose titration up to 160 mg/25 mg after 2 to 4 weeks if necessary
- May be administered with other antihypertensive drugs
Interaction
- Caution with aliskiren, digoxin, lithium, NSAIDs, corticosteroids, ACE blockers, angiotensin II receptor blockers
- May interfere with certain laboratory tests
Contraindications
- Known hypersensitivity to Telmisartan, Hydrochlorothiazide or any other component
- Do not co-administer Aliskiren with diabetic patients

Precautions & Warnings
- Caution with kidney disease, liver disease, dehydration, mineral imbalance, gout, diabetes
- May affect blood sugar and body potassium levels
- May cause dizziness
- Report prolonged diarrhea or vomiting
- Caution with the use of potassium supplements or salt substitutes containing potassium
Use in Special Populations
- Pediatric use safety and effectiveness not established
- Caution in dose selection for elderly patients
- Initiate treatment under close medical supervision in patients with biliary obstructive disorders or hepatic insufficiency
- Caution in patients with renal impairment
Overdose Effects
- Hypertension
- Dizziness
- Tachycardia
- Hypokalemia
- Dehydration
- Telmisartan not removed by hemodialysis
- Degree of hydrochlorothiazide removal by hemodialysis not established
