Indications

  • Treatment of hypertension
  • Treatment with other antihypertensive agents
  • Initial therapy for multiple drugs

Pharmacology

  • Fixed dose combination of Telmisartan and Amiodipine
  • Telmisartan: Angiotensin receptor blocker (ARB)
  • Telmisartan: Selectively blocks the AT1 receptor
  • Telmisartan: Blocks vasoconstrictor and aldosterone-secreting effects
  • Telmisartan: Dilates blood vessels and reduces blood pressure
  • Amiodipine: Dihydropyridine calcium-channel blocker (CCB)
  • Amiodipine: Inhibits transmembrane influx of calcium ion into vascular smooth muscle and cardiac muscle
  • Amiodipine: Acts directly on vascular smooth muscle to reduce peripheral vascular resistance and blood pressure

Dosage & Administration

  • Usual starting dose: 40/5 mg once daily
  • Patients requiring larger blood pressure reductions may be started with 80/5 mg once daily
  • Maximum recommended dose: 80/10 mg once daily
  • Not recommended for patients 575 years old or with hepatic impairment

Interaction

  • Co-administration of telmisartan did not result in clinically significant interaction with various drugs
  • Amiodipine has been safely administered with several other drug classes

Contraindications

  • Known hypersensitivity to the product or its components
  • Pregnancy & lactation
  • Biliary obstructive disorders, severe hepatic impairment, hypotension, cardiogenic shock, left ventricle outflow tract obstruction

Pregnancy & Lactation

  • Pregnancy Categories C (first trimester) and D (second and third trimesters)
  • No known excretion in human milk, discontinue nursing or drug use after considering importance to the mother

Precautions & Warnings

  • Avoid fetal or neonatal exposure
  • Correct any volume or salt depletion before initiating therapy
  • Monitor for hypotension
  • Slow titration in patients with hepatic or severe renal impairment
  • Monitor for worsening heart failure
  • Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker

Use in Special Populations

  • Safety and effectiveness in pediatric patients not established
  • Initial therapy not recommended in patients 75 years old or with hepatic impairment

Overdose Effects

  • Telmisartan overdose: Manifestations may include hypotension, dizziness, and tachycardia
  • Amiodipine overdose: Manifestations may include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia
  • Supportive treatment should be instituted for symptomatic hypotension
