Indications
- Treatment of hypertension
- Use as initial therapy in patients needing multiple drugs for blood pressure goals
Pharmacology
- Combination of Telmisartan and Amiodipine
- Telmisartan - angiotensin receptor blocker
- Telmisartan blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
- Amiodipine - dihydropyridine calcium-channel blocker
- Amiodipine inhibits transmembrane influx of calcium ion into smooth muscle
Dosage & Administration
- Usual starting dose of 40/5 mg once daily
- 80/5 mg dose for larger blood pressure reductions
- Maximum dose of 80/10 mg once daily
- Individualized dosage
- Add-on therapy for inadequate control
- Replacement therapy for separate telmisartan and amiodipine tablets
- Not recommended for patients 75 years or older or with hepatic impairment
Interaction
- No clinically significant interaction with acetaminophen, glyburide, simvastatin, hydrochlorothiazide, warfarin, or ibuprofen
- Possible inhibition of CYP2C19 by telmisartan
- Safely administered with several other drugs
- No clinically relevant effects on the pharmacokinetics of amiodipine with certain drugs
- No expected interaction with drugs metabolized by cytochrome P450 enzymes

- Known hypersensitivity to the product or any of its components

- Biliary obstructive disorders
- Severe hepatic impairment
- Hypotension
- Cardiogenic shock
- Left ventricle outflow tract obstruction

Precautions & Warnings
- Avoid fetal or neonatal exposure
- Correct volume or salt depletion before initiating therapy
- Titrate slowly in patients with hepatic or severe renal impairment
- Monitor for worsening heart failure
- Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker
- Initiating a CCB in patients with severe obstructive coronary artery disease may precipitate myocardial infarction or increased angina
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- Not recommended for patients ≥75 years old
- Not recommended in hepatically impaired patients
Overdose Effects
- Telmisartan overdosage manifestations: hypotension, dizziness, tachycardia
- Supportive treatment for symptomatic hypotension
- Amiodipine overdosage manifestations: excessive peripheral vasodilation, marked hypotension, reflex tachycardia
- Active cardiac and respiratory monitoring for massive overdose
