Indications

  • Hypertension

Pharmacology

  • ß1 adrenergic receptor blocker
  • Reduces heart rate
  • Lowers arterial hypertension
  • Absorbed from gastrointestinal tract
  • Renally eliminated metabolites
  • Low inter and intra-individual variability of plasma concentration

Dosage & Administration

  • Usual starting dose of 5 mg once daily
  • Dose titration interval of 2 weeks
  • Maximum dose of 20 mg once daily
  • Adjusted dosage for renal or hepatic impairment
  • No established safety and effectiveness in children

Interaction

  • Not recommended with other β-blocking agents
  • Close monitoring with catecholamine-depleting drugs
  • Caution with centrally active antihypertensive agents
  • Used with care when combined with certain antiarrhythmic agents
  • Combined use with calcium channel blockers can lead to severe hypotension

Contraindications

  • Cardiogenic shock
  • Overt heart failure
  • Second or third degree A-V block
  • Right ventricular failure secondary to pulmonary hypertension

  • No studies in pregnant women
  • Small amounts detected in the milk of lactating rats

Precautions & Warnings

  • Caution in adjusting the dose for patients with renal or hepatic impairment
  • Patients with severe anaphylactic reaction may be more reactive to allergens
  • Unresponsive to usual doses of epinephrine
