Indications
- Hypertension
- Angina
- Moderate to severe heart failure
Pharmacology
- Selective ß1 blocker
- Reduces heart rate
- Decreases arterial hypertension
- Minimal effect on lipid metabolism
- Bisoprolol is almost completely absorbed from the gastrointestinal tract
- Metabolized via oxidative pathways
- Balanced clearance between renal elimination and hepatic metabolism
- Elimination half-life of 10-12 hours
Dosage & Administration
- Usual starting dose is 5 mg once daily
- Dose may be increased to 10 mg and then to 20 mg once daily
- Dosage must be individualized to the needs of the patient
- Appropriate interval for dose titration is 2 weeks
- Safety and effectiveness in children have not been established
Interaction
- Not to be combined with other β-blocking agents
- Patients receiving catecholamine-depleting drugs should be monitored closely
- Exacerbates rebound hypertension with centrally active antihypertensive agents
- Used with care when myocardial depressants or inhibitors of A-V conduction are used concurrently
Contraindications
- Cardiogenic shock
- Overt heart failure
- Second or third degree A-V block
- Right ventricular failure secondary to pulmonary hypertension
- Sinus bradycardia

- Headache
- Gastrointestinal disturbances
- Cold or numb extremities
- Muscle weakness or cramps
- Breathing difficulties due to bronchospasm
Pregnancy & Lactation
- Teratogenic in rats at high doses
- Fetotoxic and embryolethal effects observed in animal studies
- Use only if potential benefit justifies potential risk to the fetus
- Small amounts detected in the milk of lactating rats
Precautions & Warnings
- Use caution in adjusting the dose in patients with renal or hepatic impairment
- Increased reactivity to allergens in patients with a history of severe anaphylactic reaction
- Unresponsive to usual doses of epinephrine
