Description
- Bisoprolol is indicated for hypertension, angina, and moderate to severe heart failure
- It is a selective β1-blocker with high affinity for the β1 receptor

Pharmacology
- High affinity for β1 receptor
- Reduces heart rate and cardiac output
- No change in cholesterol fraction including HDL-cholesterol
- High therapeutic reliability
- Bioavailability of 88%
- Metabolized via oxidative pathways
- Primarily metabolized via CYPSA4 with CYP2D6 having a minor role
- Elimination half-life of 10-12 hours
Dosage & Administration
- Starting dose of 5 mg once daily
- Max dose of 20 mg once daily
- Dose titration interval of 2 weeks
- Caution in patients with renal or hepatic impairment
- No need to adjust dose in the elderly unless significant renal or hepatic dysfunction
Interaction
- Not recommended with other β-blocking agents
- Monitor closely when combined with catecholamine-depleting drugs
- Use with care when used concurrently with centrally active antihypertensive agents, antiarrhythmic agents, or calcium channel blockers with negative inotropic effects
Contraindications
- Cardiogenic shock
- Overt heart failure
- Second or third degree A-V block
- Right ventricular failure secondary to pulmonary hypertension
- Sinus bradycardia

Pregnancy & Lactation

- Fetotoxic at 50 mg/kg/day and embryolethal at 12.5 mg/kg/day in rats
- No studies in pregnant women
- Small amounts detected in the milk of lactating rats
Precautions & Warnings
- Use caution in adjusting the dose in patients with renal or hepatic impairment
- Patients with severe anaphylactic reaction history may be more reactive to epinephrine
- Discuss use with doctor if having health conditions like asthma, diabetes, heart problems, liver or kidney problems, low blood pressure, or depression
