Description

Pharmacology

  • High affinity for β1 receptor
  • Reduces heart rate and cardiac output
  • No change in cholesterol fraction including HDL-cholesterol

  • Bioavailability of 88%
  • Metabolized via oxidative pathways
  • Primarily metabolized via CYPSA4 with CYP2D6 having a minor role
  • Elimination half-life of 10-12 hours

Dosage & Administration

  • Starting dose of 5 mg once daily
  • Max dose of 20 mg once daily
  • Dose titration interval of 2 weeks
  • Caution in patients with renal or hepatic impairment
  • No need to adjust dose in the elderly unless significant renal or hepatic dysfunction

Interaction

  • Not recommended with other β-blocking agents
  • Monitor closely when combined with catecholamine-depleting drugs
  • Use with care when used concurrently with centrally active antihypertensive agents, antiarrhythmic agents, or calcium channel blockers with negative inotropic effects

Precautions & Warnings

  • Use caution in adjusting the dose in patients with renal or hepatic impairment
  • Patients with severe anaphylactic reaction history may be more reactive to epinephrine
  • Discuss use with doctor if having health conditions like asthma, diabetes, heart problems, liver or kidney problems, low blood pressure, or depression
