Medicine Details

Indications

  • Hypertension
  • Angina
  • Moderate to severe heart failure
  • Not recommended for emergency treatment of hypertensive crises

Pharmacology

  • High affinity for ß1 receptor
  • Reduces heart rate and cardiac output
  • Affects lipid metabolism
  • Almost completely absorbed from gastrointestinal tract
  • Metabolized via oxidative pathways
  • Balanced renal elimination and hepatic metabolism

Dosage & Administration

  • Usual starting dose of 5mg once daily
  • Dose titration every 2 weeks
  • Caution in hepatic impairment and renal dysfunction
  • No need to adjust dose for geriatrics

Interaction

  • Should not be combined with other β-blocking agents
  • Monitoring required with catecholamine-depleting drugs
  • Exacerbates rebound hypertension with clonidine withdrawal
  • Use with care when myocardial depressants are used concurrently

Contraindications

  • Cardiogenic shock
  • Overt heart failure
  • Second or third degree A-V block
  • Right ventricular failure secondary to pulmonary hypertension
  • Sinus bradycardia

  • Small amounts excreted in the milk of lactating rats

  • Caution in adjusting dose for renal or hepatic impairment
  • Increased reactivity in patients with severe anaphylactic reaction history
