Description
- Beta-blocker medication
- Used for high blood pressure (hypertension), chest pain (angina), and heart failure
- Reduces heart rate and cardiac output
Dosage
- Usual starting dose of 5 mg once daily
- Can be increased to 10 mg and then to 20 mg once daily if necessary
- Appropriate titration interval is 2 weeks
- No established safety and effectiveness in children
- Initial daily dose of 5 mg for patients with renal or hepatic impairment
- No dose adjustment usually necessary in the elderly

- Hypertension
- Angina
- Moderate to severe heart failure
Mode of Action
- High affinity for the ß1 receptor
- Reduces heart rate and cardiac output resulting in decrease of arterial hypertension
- Minimal effect on cholesterol fractions including HDL-cholesterol
- High therapeutic reliability based on pharmacokinetic properties
Interaction
- Should not be combined with other β-blocking agents
- Closely monitor patients receiving catecholamine-depleting drugs
- Use with caution when coadministered with centrally active antihypertensive agents, antiarrhythmic agents, or calcium channel blockers
Contraindications
- Cardiogenic shock
- Overt heart failure
- Second or third degree A-V block
- Right ventricular failure secondary to pulmonary hypertension
- Sinus bradycardia
Side Effects
- Fatigue
- Dizziness
- Headache
- Disturbances of the gut (nausea, vomiting, diarrhea, constipation, or abdominal pain)
- Cold or numb extremities (e.g. hands and feet)
- Muscle weakness or cramps
- Breathing difficulties (bronchospasm) in people with asthma or COPD
Pregnancy & Lactation
- Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus
- Small amounts excreted in human milk, mothers should stop nursing if essential
Precautions & Warnings
- Use caution in adjusting the dose in patients with renal or hepatic impairment
- Patients with a history of severe anaphylactic reaction may be more reactive to allergens
