Description

  • Beta-blocker medication
  • Used for high blood pressure (hypertension), chest pain (angina), and heart failure
  • Reduces heart rate and cardiac output

Dosage

  • Usual starting dose of 5 mg once daily
  • Can be increased to 10 mg and then to 20 mg once daily if necessary
  • Appropriate titration interval is 2 weeks
  • No established safety and effectiveness in children
  • Initial daily dose of 5 mg for patients with renal or hepatic impairment
  • No dose adjustment usually necessary in the elderly

Interaction

  • Should not be combined with other β-blocking agents
  • Closely monitor patients receiving catecholamine-depleting drugs
  • Use with caution when coadministered with centrally active antihypertensive agents, antiarrhythmic agents, or calcium channel blockers

Contraindications

  • Cardiogenic shock
  • Overt heart failure
  • Second or third degree A-V block
  • Right ventricular failure secondary to pulmonary hypertension
  • Sinus bradycardia

  • Fatigue
  • Dizziness
  • Headache
  • Disturbances of the gut (nausea, vomiting, diarrhea, constipation, or abdominal pain)
  • Cold or numb extremities (e.g. hands and feet)
  • Muscle weakness or cramps
  • Breathing difficulties (bronchospasm) in people with asthma or COPD

Pregnancy & Lactation

  • Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus
  • Small amounts excreted in human milk, mothers should stop nursing if essential

  • Use caution in adjusting the dose in patients with renal or hepatic impairment
  • Patients with a history of severe anaphylactic reaction may be more reactive to allergens
