Description

AL-DS is a benzimidazole anthelmintic used in the treatment of intestinal nematode infections and in higher doses in the treatment of hydatid disease. It exhibits vermicidal, ovicidal, and larvicidal activity.

Indications

  • Treatment of single and mixed infestations of hookworm, roundworm, threadworm, whipworm, strongyloides, tapeworm, opisthorchi, and hydatid

Pharmacology

  • Mode of action: Inhibitory effect on tubulin polymerization leading to the loss of cytoplasmic microtubules
  • Mechanism: Blocking glucose uptake in susceptible helminths, leading to depletion of energy levels and subsequent immobilization

Dosage administration

  • 400 mg as a single dose in cases of various worm infections
  • 400 mg daily for 3 days in cases of strongyloidiasis or taeniasis
  • 200 mg for children of 1-2 years as a single dose
  • Not recommended for children under 1 year
  • 400 mg twice daily for 28 days in patients weighing over 60 kg for hydatid disease
  • 15 mg/kg body weight daily for patients weighing less than 60 kg in two divided doses, to a maximum total daily dose of 800 mg in hydatid disease
  • 400 mg once daily for five days in giardiasis

Contraindications

  • Not used in neonates
  • Dose reduction may be indicated in children weighing less than 10 kg
  • Not recommended during pregnancy or in women thought to be pregnant
  • No evidence to alter dose in renal, hepatic, or cardiac failure

Side effects

  • Gastrointestinal disturbances
  • Headache
  • Dizziness
  • Changes in liver enzymes
  • Reversible alopecia (rare)
  • Rash
  • Fever
  • Blood disorders including leucopenia and pancytopenia
  • Allergic shock if cyst leakage
  • Convulsion and meningism in cerebral disease

Pregnancy lactation

Should be avoided unless potential benefits outweigh the possible risks to the fetus

Precautions warnings

  • Blood counts and liver function tests before and during treatment
  • Caution during breastfeeding
  • Exclude pregnancy before starting treatment
  • Constant medical supervision and monitoring for Echinococcosis treatment
