Indications
- Long-term treatment of bronchospasm associated with COPD
Pharmacology
- Long-acting antimuscarinic agent
- Bronchodilation effect on M3-receptors
Dosage & Administration
- Recommended adult dose: 18 mcg once daily
- Only for oral inhalation with the device
Interaction
- Cimetidine increased AUC and decreased renal clearance of Tiotropium
- No significant change with ranitidine co-administration
Contraindications
- Hypersensitivity to atropine or its derivatives
Side Effects
- Most commonly dry mouth
- Other possible anticholinergic effects like constipation, increased heart rate, blurred vision
Pregnancy & Lactation
- Pregnancy Category C
- Use during pregnancy only if benefit justifies potential risk to fetus
- Not studied during labor and delivery
Precautions & Warnings
- May worsen symptoms associated with narrow-angle glaucoma, prostatic hyperplasia, or bladder-neck obstruction
Overdose Effects
- May lead to anticholinergic signs and symptoms
- No systemic anticholinergic effects seen following a single inhaled dose of up to 282 mcg
