Indications
- Hypertension
- Angina
- Moderate to severe heart failure
Pharmacology
- High affinity for ß1 receptor
- Reduces heart rate and cardiac output
- Decreases arterial hypertension
- Low inter and intra-individual variability
- Almost complete absorption from the gastrointestinal tract
Dosage & Administration
- Starting dose of 5 mg once daily for mild to moderate hypertension
- Dose titration every 2 weeks
- Maximum dose of 20 mg once daily
- Caution in patients with hepatic impairment or renal dysfunction
- No need for dose adjustment in the elderly
Interaction
- Not to be combined with other β-blocking agents
- Closely monitored in combination with catecholamine-depleting drugs
- Caution with centrally active antihypertensive agents
- Used with care when myocardial depressants or inhibitors of A-V conduction are used concurrently
- Potential risk of severe hypotension, bradycardia, and cardiac failure with combined use of β-blockers and calcium channel blockers
Contraindications
- Cardiogenic shock
- Overt heart failure
- Second or third degree A-V block
- Right ventricular failure secondary to pulmonary hypertension
- Sinus bradycardia
Side Effects
- Fatigue
- Dizziness
- Headache
- Disturbances of the gut (nausea, vomiting, diarrhea, constipation, or abdominal pain)
- Cold or numb extremities (e.g., hands and feet)
- Muscle weakness or cramps
- Breathing difficulties due to bronchospasm in people with asthma or COPD
Pregnancy & Lactation
- Potential benefit justifies the potential risk to the fetus
- Small amounts detected in the milk of lactating rats
- Use in lactating mothers essential if potential benefit justifies the risk
Precautions & Warnings
- Caution in adjusting the dose in patients with renal or hepatic impairment
- Increased reactivity to allergens in patients with a history of severe anaphylactic reaction
- Potential unresponsiveness to usual doses of epinephrine in such patients
