Dosage & Administration

  • Usual starting dose of 5 mg once daily for mild to moderate hypertension
  • Dose may be increased to 10 mg or 20 mg once daily if necessary
  • Dose titration interval of 2 weeks
  • No established safety and effectiveness in children
  • Initial daily dose of 5 mg recommended for patients with hepatic or renal impairment
  • No need for drug replacement in patients undergoing dialysis
  • No adjustment of dose required in the elderly unless significant renal or hepatic dysfunction

Interaction

  • Not recommended in combination with other β-blocking agents
  • Close monitoring required when used with catecholamine-depleting drugs
  • Caution when coadministered with centrally active antihypertensive agents or antiarrhythmic agents
  • Combined use with calcium channel blockers with negative inotropic effects may lead to severe hypotension, bradycardia, and cardiac failure

Contraindications

  • Cardiogenic shock
  • Overt heart failure
  • Second or third degree A-V block
  • Right ventricular failure secondary to pulmonary hypertension
  • Sinus bradycardia

Side Effects

  • Fatigue
  • Dizziness
  • Headache
  • Disturbances of the gut (nausea, vomiting, etc.)
  • Cold or numb extremities
  • Muscle weakness or cramps
  • Breathing difficulties due to bronchospasm in people with asthma or COPD

Pregnancy & Lactation

  • Potential risk to the fetus, consider only if benefits justify the risk
  • Not known if excreted in human milk, nursing mothers should stop nursing if essential use is considered
  • Studies in pregnant women not available

Precautions & Warnings

  • Use caution in adjusting the dose in patients with renal or hepatic impairment
  • Patients with a history of severe anaphylactic reaction may be more reactive to allergens
  • Unresponsive to usual doses of epinephrine used to treat allergic reactions
