Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain
- Cancer pain

- Pain associated with osteoporosis
Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibits reuptake of neurotransmitters (serotonin and noradrenaline)
- Modifies transmission of pain impulses
- Activates descending serotonergic pathways
- Mediated via stimulation of mu-opioid receptors
- Indirect modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses are 50 to 100 mg every 4 to 6 hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Sustained release capsule should be taken every 12 hours
- Injection dose of 50-100 mg every 4 to 6 hours
- Suppository dose of 100 mg Tramadol Hydrochloride every 6 hours for adults
- Total daily dosage should not exceed 400 mg
Interaction
- Monoamine oxidase (MAO) inhibitors should not be combined with Kingdol
- Concomitant administration of carbamazepine causes significant increase in Kingdol metabolism
Contraindications
- Hypersensitivity to Tramadol
- Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids or psychotropic drugs

Pregnancy & Lactation
- Safe use in pregnancy not established
- Tramadol crosses the placenta
- Use during pregnancy only if potential benefit justifies risk to the fetus
- Tramadol should not be administered during breastfeeding
Precautions & Warnings
- Respiratory depression may result from large doses
- Not recommended for patients dependent on opioids
- Use with caution when administering to patients receiving CNS depressants
- Use with great caution in patients taking MAO inhibitors
- Use with caution in patients with increased intracranial pressure or head injury
- Use with caution in patients with acute abdominal conditions
Use in Special Populations
- Suitable for children from the age of 1 year at a dose of 1-2 mg/kg body weight
- Suppository (100 mg Kingdol) should not be administered in children and adolescents below the age of 14 years
- Kingdol 100 mg SR Capsules not studied in children; safety and efficacy not established
