Indications

  • Moderate to severe painful conditions
  • Postoperative pain
  • Colic and spastic pain
  • Cancer pain
  • Joint pain
  • Neck and back pain
  • Pain associated with osteoporosis

Pharmacology

  • Centrally acting synthetic analgesic compound
  • Inhibition of neurotransmitter reuptake
  • Activation of serotonergic and noradrenergic pathways
  • Stimulation of mu-opioid receptors
  • Modulation of central monoaminergic inhibitory pathways

Dosage & Administration

  • Usual doses of 50 to 100 mg every four to six hours
  • Initial dose of 100 mg for acute pain
  • Initial dose of 50 mg for chronic painful conditions
  • Maximum total daily dosage of 400 mg
  • Sustained release capsule or tablet every 12 hours
  • Injection dose of 50-100 mg every 4 to 6 hours
  • Suppository dose of 100 mg Tramadol Hydrochloride 6 hourly
  • Rectal administration for suppository

Interaction

  • Contraindicated with monoamine oxidase (MAO) inhibitors
  • Caution when co-administered with carbamazepine

Precautions & Warnings

  • Respiratory depression risk
  • Not recommended for opioid-dependent patients
  • Caution when co-administered with CNS depressants
  • Caution with MAO inhibitors
  • Use with caution in patients with increased intracranial pressure, head injury, or acute abdominal conditions

Use in Special Populations

  • Dosing in children from the age of 1 year
  • Suppository not recommended in children and adolescents below the age of 14 years
  • Not studied in children, safety and efficacy not established
