Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain
- Cancer pain
- Joint pain
- Neck and back pain
- Pain associated with osteoporosis
Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibits reuptake of neurotransmitters serotonin and noradrenaline
- Modifies transmission of pain impulses
- Stimulation of mu-opioid receptors
- Modulation of central monoaminergic inhibitory pathways
Dosage and administration
- Usual doses of 50 to 100 mg every 4 to 6 hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Total daily dosage by mouth should not exceed 400 mg
- Sustained release capsule should be taken every 12 hours
- Injection dose of 50-100 mg every 4 to 6 hours
- Suppository administration for adult usual dose of 100 mg every 6 hours
Interaction
- Monoamine oxidase (MAO) inhibitors not recommended for combination
- Concomitant administration of carbamazepine requires adjustment of dose
Contraindications
- Hypersensitivity to the drug
- Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs

Pregnancy and lactation
- Tramadol crosses the placenta
- Use during pregnancy only if potential benefit justifies risk to the fetus
- Avoid administration during breastfeeding
Precautions and warnings
- Cautious administration in patients at risk for respiratory depression
- Not recommended for patients dependent on opioids
- Use with caution and reduced dosages when administering with CNS depressants
- Use with great caution in patients taking MAO inhibitors
- Caution in patients with increased intracranial pressure, head injury, or acute abdominal conditions
Use in special populations
- Children from the age of 1 year can be given in a dose of 1-2 mg/kg body weight
- Suppository (100 mg Utramal) should not be administered in children and adolescents below the age of 14 years
- Utramal 100 mg SR Capsules have not been studied in children
