Pharmacology

  • Tranexamic acid (trans-4 aminomethyl-cyclohexanecarboxylic acid)
  • Strong antifibrinolytic action
  • Low acute and chronic toxicity
  • Well absorbed by oral route
  • Inhibition of plasminogen activation
  • No interference with clotting processes
  • No tendency to thrombophilia

Dosage & Administration

  • Usual dose: 500-1000 mg 3 times daily for adults
  • Mean recommended daily doses: 0.5-1 gm orally, 500 mg by parenteral route for prophylaxis
  • Increased oral dose for therapy of hemorrhagic manifestations
  • Dosage for children based on body weight
  • No reduction in dosage for elderly patients unless evidence of renal failure

Interaction

  • Incompatible with solutions containing penicillins
  • Antagonizes the action of streptokinase & urokinase
  • Potential for increased thrombus formation with estrogen-containing drugs
  • Direct admixture with whole blood should be avoided during transfusion

Contraindications

  • Known individual hypersensitivity to the product
  • Thromboembolic disease
  • Arterial and venous thrombosis
  • Endocavitary hemorrhages
  • Serious kidney failure

Pregnancy & Lactation

  • Should not be administered during known and presumed pregnancy
  • Passes into breast milk at low concentration

Precautions & Warnings

  • Use in cases of hyperfibrinolysis
  • Prophylactic treatment 24 hours before operation
  • Therapy of hemorrhages prolonged for at least 24 hours after manifestations have disappeared
  • Caution in hematuria
  • Avoid in serious renal insufficiency or anuric syndromes
  • Particular care in cardiopathic and hepatopathic subjects
