Pharmacology
- Tranexamic acid has a strong antifibrinolytic action
- 10 times more active than conventional hemostatics
- Inhibits plasminogen activation
- Low acute and chronic toxicity
- Well absorbed by oral route
- No interference with clotting processes
DosageAdministration
- Usual adult dose: 500-1000 mg 3 times daily
- Mean recommended daily doses for prophylaxis: 0.5-1 gm orally, 500 mg by parenteral route
- Pediatric doses based on body weight
- No reduction in dosage for elderly patients
Interaction
- Incompatible with solutions containing penicillins
- Thrombolytic drugs antagonize its antifibrinolytic action
- Potential for thrombus formation with estrogen-containing drugs
- Avoid direct admixture with whole blood during transfusion
Contraindications
- Known hypersensitivity to the product
- Thromboembolic disease
- Serious kidney failure
SideEffects
- Infrequent: sense of fatigue, conjunctival irritation, nasal blockage, itching, skin reddening, exanthems
- Rare cases of postural hypotension
PregnancyLactation
- Avoid administration during known and presumed pregnancy
- Passes into breast milk to a concentration of approximately one hundredth of the concentration in maternal blood
PrecautionsWarnings
- Use in cases of hyperfibrinolysis
- Begin prophylactic treatment 24 hours before surgery
- Prolong therapy of hemorrhages for at least 24 hours after manifestations have disappeared
- Reduce doses in hematuria to prevent clot formation in the urinary tract
- Use with caution in renal dysfunctions, cardiopathic, and hepatopathic subjects
