Onychomycoses Indication

Treatment of onychomycoses, local therapy with Econazole/Triamcinolone cream, combined with an oral antimycotic, is recommended.

Effective Pathogens

  • Dermatophytes (e.g. Trichophyton)
  • Yeasts (e.g. Candida)
  • Moulds
  • Fungi (e.g. Histoplasma capsulatum)
  • Malassezia furfur
  • Nocardia minutissima

Antibacterial Effect

On gram-positive cocci and bacilli including Streptococcus pyogens and Staphylococcus aureus

Absorption

  • Econazole Nitrate: Slightly (<0.1%) absorbed from the skin
  • Triamcinolone Acetonide: Well absorbed from sites of local application

Dosage & Administration

  • Adults: Applied sparingly to the skin lesion no more than 2 times daily
  • Pediatric Use: Caution due to potential for greater susceptibility to topical corticosteroid-induced side effects

Interactions

  • Econazole: Metabolized by CYP3A4/2C9, interacts with oral anticoagulants (warfarin & acenocoumarol)
  • Triamcinolone: Interacts with various drugs such as NSAIDs, antidiabetics, amphotericin B, beta-blockers, and others

Contraindications

  • Hypersensitivity to any ingredients
  • Specific skin conditions (e.g. tuberculous, varicella, herpes simplex)
  • Decubitus ulcers
  • Rosacea and rosacea-like dermatitis

Side Effects

  • Transient local mild irritation
  • Itching & redness immediately after application
  • Adrenal suppression on long term continuous topical steroid therapy

Pregnancy & Lactation

  • Pregnancy: Topical administration of corticosteroids can cause abnormalities of foetal development
  • Lactation: Negligible amount of econazole and to some extent Triamcinolone may be excreted in breast milk

Precautions & Warnings

  • For external use only
  • Discontinue in case of hypersensitivity or chemical irritation
  • Caution for systemic effects due to absorption
  • Skin thinning and atrophy
  • Increased risk of dermatological superinfection or opportunistic infection
  • Visual disturbance may be associated with systemic and topical corticosteroid use

Overdose Effects

Corticosteroids applied to the skin can be absorbed in sufficient amounts to produce systemic effects
