Indications

  • Adjunct in the treatment of all forms of parkinsonism
  • Useful as adjuvant therapy when treating parkinsonism with levodopa
  • Control of extrapyramidal disorders caused by central nervous system drugs

Description

  • Exerts direct inhibitory effect upon parasympathetic nervous system
  • Has a relaxing effect on smooth musculature

Pharmacology

  • Non-selective muscarinic acetylcholine receptor antagonist
  • Binds with higher affinity to the M1 subtype
  • Demonstrates higher affinity for central muscarinic receptors in the cerebral cortex
  • May modify nicotinic acetylcholine receptor neurotransmission
  • May lead indirectly to enhanced dopamine release in the striatum

Dosage & Administration

  • Initial dose should be low and then increased gradually
  • Individualized dosage
  • Preferable to take before or after meals based on patient reaction
  • Abrupt withdrawal should be avoided
  • Dosage depends on the type of parkinsonism being treated
  • High doses should be divided and taken at mealtimes
  • Safety and effectiveness in pediatric patients not established

Interaction

  • Additive effects with cannabinoids, barbiturates, opiates, and alcohol
  • Increased sedative effects with alcohol or other CNS depressants
  • Intensification of anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
  • Increased risk of tardive dyskinesia with concomitant administration of anticholinergics and neuroleptics

Contraindications

  • Hypersensitivity to Trihexyphenidyl
  • Narrow angle glaucoma

  • Exacerbation of parkinsonism with abrupt treatment withdrawal

  • Constipation
  • Drowsiness
  • Urinary hesitancy or retention
  • Tachycardia
  • Dilation of the pupil
  • Increased intraocular pressure
  • Choreiform movements
  • Weakness
  • Vomiting
  • Headache

Pregnancy & Lactation

  • Use during pregnancy only when need has been clearly established and benefit outweighs risk
  • Caution should be exercised when administered to a nursing woman
  • May cause suppression of lactation

Precautions & Warnings

  • Gonioscope evaluation prior to initiation of therapy
  • Close monitoring of intraocular pressures
  • Precipitation of angle closure with increase in intraocular pressure
  • Administration with caution in hot weather
  • Anhidrosis may occur more readily when some disturbance of sweating already exists

Overdose Effects

  • Typical central symptoms of atropine intoxication
  • Neuropsychiatric signs such as delirium, disorientation, anxiety, hallucinations
  • Stupor, coma, paralysis, cardiac and respiratory arrest, and death
