Indications
- Adjunct in the treatment of all forms of parkinsonism
- Useful as adjuvant therapy when treating parkinsonism with levodopa
- Control of extrapyramidal disorders caused by central nervous system drugs
Description
- Exerts direct inhibitory effect upon parasympathetic nervous system
- Has a relaxing effect on smooth musculature
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Binds with higher affinity to the M1 subtype
- Demonstrates higher affinity for central muscarinic receptors in the cerebral cortex
- May modify nicotinic acetylcholine receptor neurotransmission
- May lead indirectly to enhanced dopamine release in the striatum
Dosage & Administration
- Initial dose should be low and then increased gradually
- Individualized dosage
- Preferable to take before or after meals based on patient reaction
- Abrupt withdrawal should be avoided
- Dosage depends on the type of parkinsonism being treated
- High doses should be divided and taken at mealtimes
- Safety and effectiveness in pediatric patients not established
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Intensification of anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
- Increased risk of tardive dyskinesia with concomitant administration of anticholinergics and neuroleptics
Contraindications
- Hypersensitivity to Trihexyphenidyl
- Narrow angle glaucoma

- Constipation
- Drowsiness
- Urinary hesitancy or retention
- Tachycardia
- Dilation of the pupil
- Increased intraocular pressure
- Choreiform movements
- Weakness
- Vomiting
- Headache
Pregnancy & Lactation
- Use during pregnancy only when need has been clearly established and benefit outweighs risk
- Caution should be exercised when administered to a nursing woman
- May cause suppression of lactation
Precautions & Warnings
- Gonioscope evaluation prior to initiation of therapy
- Close monitoring of intraocular pressures
- Precipitation of angle closure with increase in intraocular pressure
- Administration with caution in hot weather
- Anhidrosis may occur more readily when some disturbance of sweating already exists
Overdose Effects
- Typical central symptoms of atropine intoxication
- Neuropsychiatric signs such as delirium, disorientation, anxiety, hallucinations
- Stupor, coma, paralysis, cardiac and respiratory arrest, and death
